How To Access Services

Any person requesting Intake services that may be entitled to appear on the band membership list/band list receives services by the DIA where they live

(Non-member). 

Any person requesting Intake Services that may be entitled to appear on the band membership list/registry list that they notify the Agency and provide family history to process their eligibility to receive services. The intake worker shall complete the IM with the person to gather and complete a genogram.

The person must submit an application for band registry with ISC. The Elders Council, in collaboration with the Membership Clerk, determines eligibility to receive services from the Agency and forwards the information to the Agency

Any person requesting Intake services that may be entitled to appear on the band membership list/band list that they notify the Agency and provide family history to process elgibility to receive services. The intake worker salt complete the IM with the individual to gather and complete a venogram. 

The Elder's Council determines eligibility for the person to receive services from the Agency and will forward the information to the membership clerk. 

If the Membership Clerk and Elders Council have confirmed that the person’s name will be listed on ISC registry list, the Membership Clerk will then submit a letter to the Agency confirming the person’s eligibility to receive services. Only the band membership committee can confirm band membership with the person.”

Intake Responsibility – it is the responsibility of the intake worker to gather information from the potentially eligible member as well as contact information for the Elders Council and Membership Clerk to review. It is the responsibility of the intake supervisor to forward the intake to the customary care/sharing circle coordinator within 10 business days upon request if it has been determined that the person is eligible to receive services.”
